Permit Application Forms
When a property owner or project applicant wants to perform work in areas in or adjacent to Wetland Resource Areas (WRA), a permit is often required from the Conservation Commission (ConCom) to ensure that the WRA is properly protected from project impacts, or that mitigation of those impacts are properly implemented. The ConCom has jurisdiction in areas primarily defined in the Wetlands Protection Act (Massachusetts General Law Chapter 131 Section 40) and further defined in the enforcing regulations (310 CMR 10.00) and the Local Wetland Bylaws (PDF).The Town of Douglas currently relies upon the Massachusetts Department of Environmental Protection (DEP) for relevant Permit Applications Forms. https://www.mass.gov/lists/wetlands-permitting-forms

Follow the directions on the DEP web site for submitting forms to the DEP.
All applications should be submitted to the DEP first and then the Town of Douglas, this includes RDA's OOC's and ANRAD's.
Submitting applications to the Town of Douglas:
- Submit a PDF of the Plans and application to jcouture@douglas-ma.gov
- Submit two copies of the application
- Submit 2 Full size copies of the Plan or Plans
- Submit 8 copies of the plan or plans in 11 x 17
- Submit all fees for Town of Douglas and Consultant (see fee schedules)
Submitting Certificate of Compliance Applications
- Submit a PDF of the Plans and application to jcouture@douglas-ma.gov
- Submit one application
- Submit one Full Size As-Built with a red line overlay
- Submit 8 copies in 11 x 17
- Submit a Narrative from the Engineer explaining any deviations
- No Fee for this application
